Purpose and Authority

This notice is provided to ensure transparency and compliance with §12-255-137, C.R.S. and the Colorado Code of Regulations that together govern the delegation, supervision, and performance of medical or medical-aesthetic services in Colorado. It explains how such services are delegated by a licensed physician to trained and qualified individuals under appropriate supervision in accordance with 3 CCR 713-1 §1.17.

Medical Oversight

Eric Blom, MD (Colorado license number DR-43724 exp. 4/30/27) serves as the Medical Director for Juvenessence Medical Aesthetics. The Medical Director is a physician, board-certified in Ophthalmology, and qualified by education, training, and experience to perform each delegated medical or medical-aesthetic service.

Consumer Rights

Patients may verify licensure or file complaints through the Colorado Department of Regulatory Agencies (DORA): https://dora.colorado.gov/.

Delegation of Services

Under 3 CCR 713-1 §1.17, the Medical Director may delegate some medical-aesthetic services to licensed or unlicensed personnel employed or contracted by Juvenessence Medical Aesthetics.  Such delegation occurs only after documented training, competency validation, and ongoing evaluation by the Medical Director or designee

Scope of Practice & Protocols

Delegatees perform only procedures authorized by the Medical Director pursuant to standing orders and written treatment protocols maintained on file. The delegated services are routine, technical procedures that do not require the specialized skill, judgment, or licensure of a physician to perform safely and effectively under supervision. Delegated procedures may include injectables, laser/light devices, radiofrequency modalities, microneedling, peels, and other approved medical-aesthetic services.

Supervision Parameters

The Medical Director provides supervision appropriate to the procedure and delegatee’s competence, which may include direct, indirect, or general supervision as defined by Colorado standards. Real-time consultation is available during business hours by phone, secure messaging, video, or in person. Supervision level is determined by patient safety, device risk profile, and provider experience.

Medical Availability & Contact

The Medical Director is available before or after procedures for consultation, for any concerns, for complications, or for follow-up care. The Medical Director can be reached by electronic mail at director@juvenessence.net. For urgent medical concerns after business hours, patients should seek emergency care or call 911.
